Full Job Description

The Transitions Coordinator will be responsible for our 'making transitions work' programme which is funded by IGEN Trust. This includes but is not limited to, making sure all aspects of the project run to schedule, meets key milestones, record progress and final reporting to the funder.

Reporting directly to the CEO, the post holder will need to be highly self-motivated and able to work using their own initiative.

The post will involve working across different health and social care organisations including NYC, Health and Adult Services, NHS as well as liaison with schools, family carers and young people on a transitions pathway between children's and adults services.

Main tasks

· As directed by the CEO, take full responsibility for the management of 'making transitions work better.'

· Ensure all targets are met and are kept on track through regular liaison with senior managers and front-line staff across relevant departments within HS4L.

· Update SMT/CEO regularly of progress and inform where necessary of potential difficulties with meeting key objectives.

· Liaise with grant funding bodies as and when necessary and as directed.

· Carry out all surveys, questionnaires, and monitoring as per project specification and gather evidence into required formats.

· Work with business planning sub-group, board and CEO to devise future funding strategy to develop programme in years 2 and 3.

· Provide promotional materials in line with programme guidelines and contribute towards social media content as and when appropriate.

· Provide bi-monthly reports to trustees and attend meetings when requested to showcase role and developments.

· Input into future funding applications when requested.

· Work with other agencies, charities or partnerships where joint funding has been provided or is a possibility.

· Take responsibility for organisational impact measurement including use of Outcome Stars TM once fully trained.

· Work in person centred ways and develop individual person-centred plans to support the transition process.

General requirements

The post holder must at all times carry out his/her responsibilities with due regard to HS4LC policies and procedures to ensure HS4LC vision, values and strategic objectives are reflected in the work of the service.

To adhere to and fully implement the principles and responsibilities of the Equality and Diversity Policy, in all aspects and duties of the role.

Other Duties:

To maintain personal and professional development to meet the changing demands of the job, participating in appropriate training activities.

Carry out any other duties which may be required by Senior Management in accordance with the needs of the service and commensurate with the general level of the post.

  • Honours degree or equivalent level qualification.


  • Experience of working with people with learning disabilities and/or autism.


  • Willingness to work towards leadership and management qualification where deemed appropriate.

    Life transitions can be difficult for anyone but the potential for anxiety and disruption is amplified when people with long-term conditions or disabilities are being asked to move from one support setting or service network to another.

    The transition from children's services to adults' services is often very challenging for young people with a learning disability and their families: it combines a change of services and professionals at the very time when they are also negotiating wider changes to their life, for example in their educational circumstances.
