Shift: 3 days a week

Status: Part-Time

Blakeford is a senior living community in the Green Hills neighborhood of Nashville. Over the past 27 years, Blakeford has established itself as the premier provider of solutions for seniors in Nashville and the Middle Tennessee region. We provide exemplary service across the continuum of care and we are in search of exceptional people to help us maintain our standard of care and compassion.

Responsibilities

  • Serve as a medical advocate for Independent Living residents, ensuring safe, secure and appropriate level of care and educate on appropriate safe aging in the Blakeford Community.
  • Develop and manage a care plan for all residents entering the community through Independent Living, providing a streamlined mechanism for clinical care and wellness throughout life at Blakeford. Work closely with all departments to facilitate community health and wellness initiatives, educating on health initiatives and further developing and strengthening a culture of wellness for all residents in the community.
  • Attend care plans for Independent Living residents in skilled nursing and update care plan of residents with increasing needs.
  • Work directly with the Administrator of Independent Living Services and the Administrator of Health Services, along with other appropriate leaders and team members to ensure full access to services provided by Blakeford and that seniors live their best life.
  • Serve as the key communication conduit between the organization and residents, as well as their designated family members on all matters related to resident health and wellness such as medical issues, new diagnoses or medications.
  • Visit any hospitalized resident and collaborate with appropriate hospital personnel to assure smooth transition back into the Community at the appropriate time with full access and availability of any needed services.
  • Work with the community marketing team to ensure appropriate intake and assessment of all health information pertaining to future residents in the community.
  • Cultivate and maintain professional relationships with the physical therapy team, wellness and life enrichment teams, hospital discharge planners, Blakeford's clinical care team and social worker, as well as other community outreach programs and healthcare professionals.
  • Collaborate with internal teams to ensure smooth implementation of clinical programs.
  • Advise executive leadership team on any health issues or concerns observed while completing day to day assignments and tasks.
  • Assist with discharge plans and summaries as needed.

Education And Experience

Bachelor’s degree from a four-year college or university and three to five years related experience.

RN License

COMMUNICATION: Excellent communication and presentation skills
