HOME HEALTH INTAKE COORDINATOR: YOUR ROLE
As a Home Health Intake Coordinator, your day will begin with reviewing new referrals and reaching out to families to welcome them to our program. You will work closely with families, guiding them through the intake process, and collaborate with our insurance team to ensure all required information is collected and verified for service approvals. Your role involves meticulous management of documentation to facilitate a seamless transition for new clients. Each day, you will be the reassuring contact that helps families feel supported and informed as they embark on their journey with us.

Previous Intake Experience Is Preferred But Not Required.
YOUR HOURS
This full-time position follows a standard schedule of 9 AM to 5 PM, Monday through Friday, providing a stable routine with weekends off.
