"🔬 Highlighting key findings from our latest study on cirrhosis and heart electrical activity: 📊 Study included 425 patients, median age 36, with 57.6% being male. Cryptogenic and primary sclerosing cholangitis were the most common causes. 🫀 Notable ECG findings: Prolonged QT (24.7%) and early transitional zone (19.8%). These changes linked to cirrhosis cause and Child-Pugh class. 👩⚕️ Concluding that a prolonged QT interval and early transitional zone may signal cardiac dysfunction in cirrhosis patients, emphasizing the need for further evaluation. As a foreign body was suspected in the patient, the frontal chest radiographs were obtained in inspiration and expiration. There was no significant difference in mediastinal shift on the two frontal chest radiographs with different levels of inspiration, making the possibility of a foreign body unlikely. The right lung is mildly hypoplastic, with the trachea bent to the right and shows diminished vascularity. The left lung shows increased vascularity. The right heart border is not obliterated. Axial MR image shows that the right pulmonary artery is absent in the mediastinum and the right lung surrounds the right heart margin,this finding is in contrast to that seen in most scimitar syndrome cases in which the right lung does not surround the far right margin of the heart. Ending the week with this one 😵 A 60 y F presented with a NSTEMI. Had chest pain for a day before coming to hospital. ECG demonstrated widespread T wave inversion with elevated Trop I levels. Echo was performed at bedside before referring to the cardiac facility and demonstrated • apical wall hypokinesia. Hypokinesia of the mid anterosetpal wall can also be found • a mobile apical thrombus with high like of embolization. • lvef was slightly impaired •no other ischemic echo findings found. Coronary angiogram found triple vessel disease. 1)The left lung volume is reduced, with the heart and mediastinal structures displaced to the left. 2)The left diaphragm is elevated. 3)The intercostal spaces of the left thorax are mildly narrowed. 4)The pulmonary vascularity is reduced in the left lung and increased in the right lung. Patients with silent myocardial ischemia, or silent MI, are at risk for poor outcomes.
