But as #homelessness #rates #increase #across the #country – #exacerbated by a #mentalhealth and #addictions #crisis – #ERs have become almost an #inevitable #destination for #Canada’s most #vulnerable. #Fearing where things are headed, a #chorus of #doctors are #calling for #radicalchange: making the #case, to whomever will hear them, that #safe, #affordable, #secure #housing is the #best #prescription for their patients. #Alternative #community #healthcare supports have long #existed at the #grassroots level. But now, #physicians and #hospitals are also beginning to look for ways to address the #root #social #causes of their patients’ #illnesses and #injuries. Some are setting up #satellite #services outside of the health care #facilities to #provide #carenear #shelters, #parks and #tent #encampments. Others are going so far as to #provide #housing itself. Harjholi Jhojha, a small village in #Haridwar, faced severe challenges in the space of #publichealth. With facilities miles away and a prevalent mistrust of institutional deliveries, most births occurred at home, significantly increasing maternal and infant mortality risks. Home deliveries accounted for 42% of births in 2021, a stark contrast to national and state averages of 11% and 17% respectively (National Family Health Survey (NFHS-5), 2019-21). In 2021, #AIF launched #MANSI 2.0, in collaboration with Rural India Supporting Trust (RIST), in Haridwar to improve maternal and neonatal health outcomes in rural communities. AIF assigned Neha, an Adolescent Community Health Facilitator (ACHF), to take charge in Harjholi. Partnering with local Accredited Social Health Activists (ASHAs), Pooja and Sushila, Neha embarked on a mission to combat these deep-rooted fears, financial constraints, and structural obstacles to lower #infantmortality rates. Neha and her team trained #ASHAs, counseled families, conducted joint home visits, and held community meetings to highlight the importance of institutional deliveries. Engaging with respected local dais (birth attendants) was crucial, and through persistent dialogue, they encouraged dias to refer complicated cases to healthcare facilities. The results have been nothing short of inspiring and–literally–life saving. By 2022, institutional deliveries in Harjholi Jhojha soared to 81%, and by 2023, they further increased to 85%, surpassing state and district averages. Out of the infant mortality rate, the neonatal mortality rate has also dropped from 89% to 71% since the launch of MANSI 2.0 in 2021. This remarkable shift is a testament to the power of persistent, community-driven efforts and the critical role of local partnerships. Tackling inequalities in elective waiting lists We need to work towards reducing health inequalities as this is just and fair. People in deprived areas wait twice longer compared to those living in affluent communities and due to various factors that are unfavourable those on the waiting list from deprived communities rapidly deteriorate and suffer the consequences thus creating a circle poor health and poor wealth. A very interesting King's Fund report which has addressed the issue of addressing inequalities in elective waiting lists. Taskforce - consultation on the Aged Care Funding Principles The Aged Care Taskforce will review funding arrangements for aged care and develop options for a system that is fair and equitable for everyone in Australia. This initiative underscores the importance to all Australians of reforming funding for aged care. This included the need to address the long-term funding and financing challenges of the aged care sector to ensure that older people have access to an equitable, innovative, high-quality aged care system both presently and in the years ahead. How can I contribute to stigma reduction among TB-affected individuals?   I had this question in my mind when I learned about the stigma faced by vulnerable communities in the society. However, I got my answer after receiving an opportunity to attend the launch of ‘The Accountability Report of TB-affected Communities & Civil Society: Priorities to close the Deadly Divide’ by REACH India and STOP TB Partnership. One of the calls for action in the report entailed making the TB-affected communities and civil societies stigma-free by 2025. Non-surprisingly, a TB champion who attended the event said that existence of stigma leads to non-compliance of treatment among TB-affected people in slums and rural areas.   As members of the community, we can contribute significantly towards improvement of health services by TB patients through behavioural change activities. One of these could be creation and display of educational videos about the disease, which would not only lead to awareness generation among the vulnerable population regarding TB but would also increase confidence of TB patients and their families.   Another small step could be taken in the form of providing psychological support to the patients and their family members, so they do not feel discouraged and disheartened upon the diagnosis. One-to-one or group counselling sessions could be done with the needy to sensitize them and address their doubts. A safe environment should be created for the TB-affected individuals so that they can be encouraged to disclose their status and receive appropriate treatment and care.   Being a part of the community, our role is crucial for the synchronization of tuberculosis related health activities in the society, and these small individual efforts can pave the way for early disease detection and treatment.
