We are starting our first trauma registry! Trauma is a leading cause of morbidity and mortality globally, with a disproportionate burden affecting low- and middle-income countries (LMIC). Our reason for embarking on this strategic journey is driven by our goal to analyze the impact of trauma and the nature of injuries in our hospital based patient population and nationally so that we and our healthcare partners can develop quality improvement initiatives that prevent injury, optimize trauma care and provide vital information to policy makers on resource allocation and injury prevention programs. Recommendations for a successful trauma registry implementation: ✅Strategic priority with buy-in from policy makers, institutional leaders, individual patients and the community. ✅Adequate and sustainable funding for both start up and annual operating costs. ✅Trauma registry training and specialized personnel. ✅Customized, standardized and inclusive data collection tool supported by Trauma registry software and hardware. ✅Quality patient records and an effective strategy for patient identification and documentation. ✅Project team made up of key stakeholders #peoplecentredcare #serviceexcellence #strategicpartnerships #TCIhospitalstrong

Leeds Teaching Hospital have issued a PIN notice to enter market engagement with private hospital operators interested in investing in and operating private patient services at St James Hospital. Whilst there are a number of successful NHS run PPU's, these types of partnership can bring benefits to all stakeholders: - The NHS partner secures a profit share without capital investment (very few trusts will prioritise using CDEL to grow private patient services) and accesses the commercial skill/ PMI reimbursements of a private operator - The private operator can undertake more complex/ higher acuity procedures than on a stand-alone hospital site - It is more convenient for consultants, allows them to offer more complex procedures, and PP capacity is contractually ring-fenced - Patients have immediate access to the medical team and other services found on a large acute hospital There are a number of very successful precedents for this model (eg Christie, UCLH, Guy's, Birmingham, as well as smaller cardiology and oncology examples - Latchmore have produced a free report summarising these)

Happy Monday! During my hospital shift yesterday, one of my coworkers shared an article with me that had been published in The New Yorker a month ago. The article discusses ECMO (Extracorporeal Membrane Oxygenation) and its impact on healthcare. Having spent most of my nursing career in critical care, I've become very familiar with this treatment. I've seen patients cannulated with ECMO during cardiac arrests and worked with those who survived COVID-19 because of it. While some patients make miraculous recoveries, others remain in limbo. When managed properly and combined with other modalities, this treatment can sustain life for an extended period of time, which brings me to question its ethical implications. As healthcare providers in the US, we often push the envelope a bit too far. However, it's also how groundbreaking treatments like ECMO are developed. Over the years, I've witnessed how much we can push medical boundaries. Although I fully support medical advancements, they should never compromise quality of life for quantity. Many patients do not have the opportunity to make an educated choice when it comes to this type of treatment because they can never understand what life looks like tethered to a machine without any real possibility of recovery until they experience it firsthand. ‘This trial is about so much more than just numbers – it's about redefining what's possible in healthcare and making a lasting impact on patients' lives.’ Diane Mackle, an experienced ICU nurse and now Project Manager of the Mega-ROX trial at the Medical Research Institute of New Zealand (MRINZ), plays a pivotal role here in New Zealand managing the study and its coordination with the Spiral team and ICUs across the globe. The trial, aiming to compare conservative oxygen therapy with usual oxygen therapy in mechanically ventilated patients intubated in the ICU, is designed to determine which approach could potentially reduce mortality compared to the traditional liberal approach. We asked our community how long it took to receive their diagnosis of myalgic encephalomyelitis (ME, aka ME/CFS). The answers came in quickly and almost every one was measured in years- not months. And these are those able to get a diagnosis. It has been estimated that around 90% are undiagnosed (NAM report). We are working hard to change that scenario for all those who are yet to be diagnosed and those who are diagnosed but need knowledgeable care. #MEAction’s annual #MillionsMissing campaign has a theme of Teach M.E.
