Please clarify if you mean using the Notifications API and Push API (which allows displaying operating system notifications even if the user is not using the website at the moment), or just updating the existing notifications icons/menu inside the website without reloading the page?
Either way this should be done for all Echo notifications, not just those generated by DiscussionTools.
I found existing tasks about both approaches:
- T113125: Investigate using service workers to provide real-time Echo notifications in the browser (push notifications) – Notifications/Push API, has a work-in-progress patch from 2017
- T34284: Update Echo Notifications in real time without page reloads – Updating the icons/menu, looks like this is complete and working on the beta cluster since February 2020, but hasn't been deployed to production wikis
