About us
Community Care Cooperative (C3) is a 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by Federally Qualified Health Centers (FQHCs) in Massachusetts. In 2016, leaders from 15 FQHCs throughout the state formed C3. An innovative model for an Accountable Care Organization (ACO), C3 leverages the proven best practices of ACOs throughout the country. C3 is the only ACO in the state founded and governed by FQHCs exclusively focused on advancing integrated and coordinated community-based care for MassHealth members. C3 is building its organization on the collective strengths of health centers. We currently have 17 partner health centers across the state and our continued growth and expansion enables C3 to better serve MassHealth members across the Commonwealth.

To our partners, The cybersecurity vendor called CrowdStrike published a software patch that has caused widespread outages to multiple IT infrastructures. Every indication at this time is that this is an error, not a cybersecurity attack. At C3, while we do not use CrowdStrike, some of our technology partners do. Therefore, some applications may be unavailable, and others may have degraded performance. The situation is evolving rapidly. However, these outages are occurring at the highest levels of multinational technology companies like Microsoft, and these companies will need to implement solutions to these problems in their systems and technology to ultimately resolve any degraded status of C3's collaborators’ systems. If you are experiencing any difficulties because of this and we can give you assistance, please let us know. Meet our Board Members! Tracey Cohen, MD, FASAM is the Chief Medical Officer for New Bedford Community Health Center! "Dr. Cohen’s career has ranged from practicing primary care and addiction medicine to leadership roles in community health centers, top health plans, a multi-state integrated care organization, and a health technology start-up. Before joining New Bedford Community Health Center, she served as Interim Chief Medical Officer and Senior Medical Director at Point32Health. There she led Point32Health to become the first plan in New England to achieve NCQA’s Health Equity Accreditation. During her time as Medical Director at Neighborhood Health Plan of Rhode Island and Blue Cross Blue Shield of Rhode Island, she partnered with the Rhode Island Executive Office of Health and Human Services and other state agencies in the development of programming and policies that expanded access to holistic models of care, addiction treatment, safer pharmacy utilization, and LGBTQ care including gender-affirming treatment." A big thanks to Colleen Shanley-Loveless MBA - She/Her and the RCDC team for their partnership through Flexible Services! Together we are supporting hundreds of our health center members with healthy home goods - and improved health outcomes! Lack of affordable housing, risk of eviction, poor housing quality, and homelessness are all-too-common barriers to health equity for our members. We appreciate the opportunity to come together with our health care colleagues from the Western MA region to highlight the importance of #HousingforHealth.

To all of our providers, We are currently looking to add members to our very active Patient & Family Advisory Council (PFAC), which is a partnership between us and our patient and family member advisors, dedicated to advancing comprehensive and compassionate patient and family-centered health care for the myriad diverse populations all of us serve. The PFAC meets quarterly, with the express goal of improving the patient and family care experience by identifying opportunities, gathering, and providing feedback and perspectives on the plans, activities, consumer-oriented materials, and programs of C3 related to patient and family-centered health care. What are the responsibilities of PFAC members? - Partner and collaborate with fellow council members, as well as C3 staff, patients, and families, to ensure a patient and family-centered care experience. - Facilitate changes in care where, and when appropriate, to support the achievement and continued improvement of patient and family-centered care. - To always provide constructive feedback, from a PATIENT and FAMILY perspective. - Listen to the diverse opinions of others. - Adhere to the Confidentiality Agreement. - Review the materials received prior to each meeting, so you will come prepared to ask us questions, give us YOUR ideas, and provide us with YOUR input. - Make decisions based on consensus. When consensus is not reached, decisions will be made by a majority vote of ALL members. - A willingness to serve on the board for a minimum of two years. - Attend a minimum of three meetings a year. (There are only 4 in total.) - Have a willingness to serve on sub-committee activities when necessary. For every meeting attended, all PFAC members will receive a $75 gift card.
