Upham’s Community Care offers medical, dental, vision, elder, and urgent care; WIC; home health care; and PACE (Program of All-Inclusive Care for the Elderly) to North Dorchester and its surrounding neighborhoods in Boston, Mass. This care is tailored to meet the cultural, financial, language, and access needs of our vibrant and diverse community.     Jagdeep Trivedi (Jay), CEO of Uphams Community Care, was a latch key kid raised in Medford, MA. His parents emigrated from India in 1973 with only $8 to their name, but they worked hard to secure jobs in the medical field in to support their three sons. Inspired by his Mom & Dad, Jay wanted to pursue a career in Neurology, but since his grades didn't cut, he had to take a different path. Today, Jay Trivedi, is only the second CEO in the 53 year history of Upham's Community Care. How did he get there? It wasn't easy... We gathered to focus on the critical issue of the "primary care access crisis." The day featured a legislative briefing packed with valuable insights, followed by lunch in the Great Hall and an engaging awards ceremony. We were honored to have many of our community health partners present, rallying together to support this vital cause.     Our CEO, Jay Trivedi had the privilege of joining Governor Healey and Lt. Governor Driscoll at a round table discussing the vital link between housing and public health. The discussion focused on how the high cost of housing affects healthcare recruitment, workforce retention, and ultimately, patient outcomes. Governor Healey emphasized that affordable housing is a cornerstone of public health and equity, highlighting the Affordable Homes Act's potential impact on residents' well-being. Lt. Governor Driscoll echoed the sentiment, emphasizing the urgent need to address housing as both a workforce and public health issue. With input from industry experts, the dialogue shed light on the pressing challenges faced by healthcare providers and communities alike. Mr. Trivedi is quoted, “I'm proud to emphasize the critical link between housing and health, especially for seniors. Despite the synergy between Programs of All-Inclusive Care for the Elderly (PACE) and housing, Boston faces a shortage of affordable units, prompting our advocacy efforts. Through initiatives like co-located supportive housing units and partnerships with developers, Upham's PACE bridges healthcare and housing.     Our pharmacy grand opening was a success! In addition to remarks given by our CEO Jagdeep Trivedi, we also heard from Upham's Pharmacy Director Marina Rabinovich, State Representative Christopher Worrell, and Upham's Chairperson Margaret Leahy Wirth. At Upham's Community Care, our mission is always to serve our community with compassion, integrity, and excellence in healthcare services. As pharmacy chains close, Upham's Pharmacy is one of the remaining independent pharmacies in Roxbury, Mattapan, and Dorchester that continues to meet the needs of the community. Upham’s Pharmacy has been a pillar of the community for over 14 years; an updated facility and new pharmaceutical services represents a significant milestone in our commitment to provide equitable, easy-to-access, high- quality healthcare solutions to the residents of Dorchester and adjacent neighborhoods.     🌟 Breaking News! 🌟 We are thrilled to announce that Upham's PACE has been designated as "Age-Friendly" by the city of Boston! 🎉👏 After meticulous walk-through visits at of our PACE Centers, the City's staff assessed various age- and dementia-friendly features and services we offer, including bright lighting, accessible facilities, and door-through-door transportation. This designation is a testament to our unwavering commitment to providing exceptional care and support to the elderly in our community. In the words of our CEO, Jay Trivedi, "This certification reaffirms our dedication to creating environments that prioritize the needs and well-being of our elderly population. We are proud to be recognized by the city of Boston and will continue to strive for excellence in our services."     The Massachusetts Pharmacist Association is the leading organization, advocating for the pharmacy profession in Massachusetts, representing all pharmacists in all practice settings. To elevate the awareness of pharmacists we are spotlighting pharmacists from all around the state. Meet Holly Knight, PharmD RPh Tell us about yourself: I started out my pharmacy career with a GED and worked my way up to become a certified pharmacy technician for many years in both retail and hospital pharmacy. I was encouraged and inspired by many folks along the way (shout out to Kelly Barnes, Joanne Doyle Petrongelo, and Juanita Bukenya, to name a few). I received my undergrad from RIT and transferred to Northeastern University to complete my doctorate. I am married to an amazing man who is devoted to community, health equity and racial justice. We have a smart, funny, poetic and kind 10-year-old daughter. We enjoy being with our large extended family, traveling and being on Martha’s Vineyard in the summer. Tell us about your current practice setting: I practiced for 10 years as a community pharmacist in The Bronx, Cambridge and Upham’s Corner in Dorchester. I then became a field leader in retail pharmacy, leading up to 25 pharmacies. I prided myself on supporting my pharmacy teams in underserved communities in and around Boston. After many years and many changes in the pharmacy profession, I began to reflect. I left my position as a district manager, took some time off to reset and enjoy family, and I am now working at Upham’s Community Care in Dorchester. I am so happy to have found the ideal job for me! It is such an incredible team of clinicians and support staff. I am proud of the work we do every day. I love reconnecting with my former patients in Upham’s Corner (some hug filled reunions!), getting to know new patients, and being a part of a collaborative team that improves lives by chipping away at barriers to health each and every day. I also enjoy the absolute simple pleasures that this change brings: driving my daughter to and from school, working through homework questions, cooking dinner and being together with my family every weekend. I am no stranger to recreating myself and this recent career “glow up” was the best decision! I renewed my love of pharmacy. #highlight #whenigrowup #pharmacist
